An item with same key has already been added

I'm running into an error on my K2 Appit where my list is saying An item with the same key has already been added.

 

On the Application Page the list of SmartObjects, Views and Forms is completely blank.

 

I tried going into the K2 Management and edit my form but I get the same error.  A co-worker attempted to but it will not allow him to because the view / form is checked out to me.  

 

The error occured when I tried created a hide/show rule on my save button.   

 

I've looked at the forums and found some people said it was a duplicate row (not the case) and to go into the SQL and run a script to force the check in but I don't see a SQL Table that matches the k2.form.auditlog.

 

** Update - I went to the K2 Management and got the same server error but now can't access any of the site collections because it's populating the same error.  Uploading more screenshots.

 

Any help would be greatly appreciated!

I would recommend opening a K2 Support Ticket to get this resolve.
